Family Practice Associates employs a Certified Diabetes Educator to meet with patients who are pre-diabetic, newly diagnosed, or have been living with diabetes for many years. Jessica Rich, RN, teaches patients lifestyle habits that can help them control the health impact of their disease, including exercise, nutrition, medication management, and how to check blood sugar levels. She can also assist patients to set up glucose monitors and help answer many questions that both patients and family have.

Patients and their families may meet with Jessica one-on-one or your provider may ask Jessica to meet with you during a routine wellness visit. Jessica can also provide insight and expertise into how your diabetes, or your medication may affect other chronic conditions.
